SKILLED NURSING

After a hospitalization, an office appointment, new medication or a change in condition, your physician may order Home Health Services. In most cases a Nurse will be sent to your home to do an initial assessment and work with your doctor to determine the best plan of care.

 

At Gallagher Home Health Services, we use a Primary Care Model, which means you will have one nurse assigned to you to ensure that your care is well coordinated. Our patients feel more comfortable when they know the same nurse will show up at their door for each visit. We believe our team of highly skilled and compassionate nurses are the best in the business and so do our patients.

 

Our Nursing clinicians may provide one or more of the following services:

·         ASSESSMENT & OBSERVATION

·         WOUND CARE

·         INJECTIONS and/or INFUSION THERAPY

·         CATHETER AND OSTOMY CARE

·         TEACHING AND INSTRUCTION
