Skilled Nursing Details:

 

ASSESSMENT & OBSERVATION

·         Skilled patient assessment of your needs

·         Blood Pressure, Pulse, Respirations, Temperature

·         Checking your Pulse oxygenation

·         Review of your Medications and their purpose


PERFORMANCE OF SKILLED PROCEDURES


WOUND CARE

·         Assessment of wounds and the response to treatment

·         Dressing changes including wound packing

·         Contact with your physician or wound care center to ensure continuity of care

·         Wound VAC application and management


INJECTIONS and/or INFUSION THERAPY

·         Perform Intramuscular or subcutaneous injections such as Insulin, Lovenox, B12, etc.

·         Care and Management of your PICC line (Peripherally inserted Central Catheter)

·         Care and Management of your PORT

·         Chemotherapy

·         IV Antibiotic, Antiviral, and Antifungal therapies

·         IVIG Therapy

·         TPN (Total Parental Nutrition)

·         Hydration Therapy

·         Pain Management

·         Venipuncture and management of lab services


CATHETER AND OSTOMY CARE

·         Changing indwelling catheters

·         Management of catheters

·         Irrigating catheters

·         Care of new ostomies, ileostomies, stomas


TEACHING AND INSTRUCTION 

·         Evaluate knowledge base of patient and family members related to current episode of illness

·         Instruct patient and family in new or changed medications

·         Instruct patient and family in any procedures that they are willing and able to learn

·         Instruct in what symptoms to report to physicians and when.

·         Instruct patient and family in new disease processes and how to manage them and answer any questions or concerns you or your family may have.
